How to apply for Unique Disability Id card online

 


Hello friends, in this blog we are discussing about how to apply for Unique Disability Id card (UDID) online. If there any psychically disabled person in your family or in your friend circle and you want to apply for UDID card then you can easily apply for it.

 

In previous time many of disabled persons were issued disability certificate. Now the disability certificate is nationalized by the government. Now you need to upload the old certificate and can get a new Unique Disability Identity card online.

 

Those who have no old certificate they can also apply in this new portal online. After applied online disabled person will be check by doctor in district hospital or in any other institutions and there after they approved the certificate. Once the process is completed successfully the UDID card can be downloaded online.

 

How to apply Unique Disability ID card online:

Before going to apply for UDID card you must keep some things handy. You will be needed a passport size photograph within 3 to 30 KB, photo of signature or thumb impression within the size of 3 to 30 KB, Aadhaar or any address proof or identity proof card’s photo within the size of 100 kb etc. After keeping all this documents on your PC or mobile phone starts the following process. In the following steps we are discussing how to apply for Unique Disability Id card.

 

STEP 1: Go to the website https//:www.swavlamancard.gov.in and open the home page of the website.

 

STEP 2: As the home page of the website open you will see a link ‘Apply for Disability Certificate & UDID card’ on the right side bar of the page.

 


STEP 3: In this page a form will be appeared. In this form you have to enter the first name, middle name and last name of the applicant. You need to enter date of birth, gender, father’s name, mother’s name, mobile number, email Id, category, marital status, care taker/ guardian’s name, blood group etc. of the PWD person.

 

You also have to enter relation of guardian with PWD, mobile number of the guardian, communication address and permanent address, education qualification etc. of the person with disability. Enter all the details correctly upload a passport size photo of size within 3 to 30 kb, upload a photo of signature or thumb impression within the size of 3 to 30 kb. You need to select and upload an address proof Id within the size 10 to 100 kb.

 

After completing all the required details of the form click on ‘Next’ button.  

 


STEP 4: Another new page will open. In this page first you have to select if you have any disability certificate or not. If yes then you also need to enter the details like issuing authority, registration number of the certificate, issuing date and also need to upload a photo of your disability certificate within the size of 10 to 100 kb.

 

If you don’t have any disability certificate then select the option ‘No’ and select the disability type, disability since or disability by birth, disability area, hospital treating disability, hospital name, disability due to etc. Enter all the details correctly and click on ‘Next’ button on the bottom of the page.

 


STEP 5: You will be redirected to another page. In this page you have to enter employment details if any. If the PWD is employed then enter employed otherwise not employed. Enter occupation, enter if you are under APL/BPL etc. After entering details you click on ‘Next’ button on the bottom of the page.

 

 

STEP 6: In this new page you are asked to select identity proof, Enter your aadhaar number, trick on the box before ‘I agree to share Aadhaar information with govt. department’. Enter the captcha code and agree, trick on the box before ‘I have read and agreed and finally click on ‘proceed’ button.

 

 

STEP 7: You will be redirected to a new page where you will see your filled up form. Check the form as you want and click on ‘confirm application’ and click on ‘edit application’ to make correction of the application.

 

STEP 8: A pop up window will be appeared. Here you will see some questions. Read the question and click on Yes for every question answers and finally click on the ‘Yes’

 

STEP 9: Now your form will be submitted successfully. The enrolment number will also be shown. Note the enrolment number for further use. You can also download the application form receipt by clicking on download receipt option.

 

After submitted the application take a print out of the application and submit it at your district medical authority. After submitted the application disabled person will be call and checked up by doctor.
